Datatel User Account Form (STUDENT SYSTEM ONLY)

Datatel DISTRIBUTED FINANCIALS User Account Request Form

All Datatel forms require a minimum of three signatures.

• Supervisor/Department chair
• Appropriate Module Administrator from each area (Listing located on page two (2) of the forms.)
• Employee

NOTE: If you require Datatel access to both the Student System and the Distributed Financials, you must complete both forms and have the appropriate signatures on both forms.
